HERE’S HOW THIS REPORT WORKS:

STEP 1

You’ll take the survey above. Please answer the questions in it as openly and honestly as you can. Your answers will be anonymous.

STEP 2

After every member of your team finishes the survey, you will receive an email with a private PDF report for you containing your own personal scores. This report will not be shared with anyone but you.

STEP 3

Your team leader will receive a roll-up report containing the anonymous data aggregated from your and your team members’ scores. There will be no way to tell what you personally scored, but your team leader will be able to see overall trends and opportunities for growth—plus trends that are cause for celebration!
